Article 9

All Providers

Article 9 is offered as an online, self-paced course. Please reach out to your Area Supervisor to get registered for the course. You will be e-mailed further instructions for completion when signed-up.

CPR/First Aid

Tucson

For class schedules, please visit https://cpr2uaz.enrollware.com/schedule#ct14467 and take a look at their calendar. Please ensure you are specifically looking at the HeartSavers CPR/AED and First Aid class. Once you find a class that you can attend, please email your Area Supervisor the date/time of the class you are interested in. Please refrain from registering yourself for the course.

Phoenix & Tempe

For class schedules, please visit https://ap.training/ and take a look at their calendar. Once you find a class that you can attend, please email your Area Supervisor with the date/time of the class you are interested in. Please refrain from registering yourself for the course.

Coolidge

To schedule a class, please call or text Ramon Lopez at (520) 858-6631. Please notify him that you are a provider with Absolute HCBS and need to be recertified in CPR/First Aid.

Fingerprint Clearance Card

All Providers

You can go to https://psp.azdps.gov/ to apply for a card. Please read through our detailed instructions to assist in this process.

Once your prints are completed, an email confirmation with your application number will be sent to you. Please forward this email to your Area Supervisor.

Please be sure to send a copy of the front and back of your card once you receive it.

These must be done before the expiration date on your card to keep you in compliance with the State. We don’t cover the cost of fingerprints as this is a responsibility to maintain as a provider.

Criminal History Affidavit

All Providers

This document must be notarized. Each “box” on the form must be individually checked. If this document is submitted with a line going from top to bottom through the check boxes, it is considered invalid and will be rejected. Please email the completed document to your Area Supervisor.

Transportation Permission / Decline Document

All Providers

The Transportation Permission/Decline must be updated annually for each client served.  If you are providing transportation to a client, please complete the top portion of this form.  If you are not providing transportation to a client, please complete only the bottom portion of this form. Please do not fill out both the top and the bottom of the form. Once completed please email to your Area Supervisor.

Upcoming CEU Expiration

ATC Providers

Direct Care Workers who provide Attendant Care must complete six hours of CEU’s per year by the expiration date.  The Continuing Education Units are similar to those which teachers and social workers must take in order to maintain their state certification.  Some classes like Prevention & Support or Article 9 count for these units as well.  Please keep in mind that these classes must pertain to our field of work. 

For classes please go to https://ap.training/ and take a look at their calendar.  We have to sign you up for their classes.  Please email your Area Supervisor which classes you would like to take and she will get you registered. 

Please note that CPR and First Aid do not count towards CEUs.

To receive compensation for time spent attending training please print off a Training Time Sheet from our website and submit it with the next pay period.

Please keep in mind if you are due for Article 9, Prevention & Support, currently take any other CEU’s, or if you are a college student and attend classes related to our field in any way, these will suffice for this requirement.  You will simply need to provide proof of attendance and passing the course.

This Continuing Education Unit requirement is necessary to remain an ATC Provider and temporary suspension will be necessary if they are not complete.

Motor Vehicle Records

All Providers

To obtain a copy of your record please visit https://azmvdnow.gov/home and follow these instructions.  Please be aware that there is a $3 fee for this record.  

Please be careful and enter information correctly.  If information is not entered correctly it could result in a “NO RECORD FOUND”.  We will not accept a “NO RECORD FOUND” report.

Once you have received your copy, please send a copy to your Area Supervisor by the expiration date to avoid suspension.  
If you are no longer transporting clients, please return the Transportation Permission/Decline & Auto Safety Checklist with the bottom portion signed by both provider and client guardian (you will need one for each client) and email it to your Area Supervisor.

Annual Review

All Providers

This training is required for all Absolute HCBS providers, and it must be completed annually. In order remain in compliance with local, state, and federal employment laws; as well as our AHCCCS and DDD Qualified Vendor Agreement, this training encompasses company policy and procedures, HIPPA and privacy, cultural competency, electronic visit verification, and abuse and neglect.
