Person-Centered Planning is a plan that gives an outline for each consumer. It is highly individualized and involves many hours of interviews to map out where the individual has been and where the individual envisions themselves as they grow older. Person-Centered Planning ensures that the individuals wants, hopes, dreams and needs are being addressed in service delivery.
Person-Centered Plans include details about the individual and require input from a variety of people involved in their lives, such as family, friends and teachers. It is important to invite your providers and Area Manager/Supervisor to the annual ISP meetings with your DDD Coordinator. Together, the team will ensure that service delivery is person-centered and addresses areas that are important to the individual.
